Welcome to Tobyhanna Army Depot Child & Youth Services program. Registration is now made easy. There are a few simple steps you need to take.

Step 1. Based on which program you are registering in, complete the registration forms noted below.

Child Development Center Enrollments: Forms #1, #2, #3, #4
Infant Parents: Please review #5 Infant Menu-Parent Forms

School Age Services Enrollments: Forms #1-4 (all pages).

Youth Services Enrollments: Forms #1; (#4 is needed if under age 13).

The remaining forms need to be completed, printed, and brought to registration.

* Form #2 ‘Health Assessment’ must be printed, completed then signed by your doctor.

Step 2. Make an appointment with Central Enrollment Office at (570) 615-6559. 

Step 3. Bring all of the completed Forms (your registration packet) to your appointment.  During your appointment a review of all your forms will be reviewed for accuracy and completeness.  You will then sign and date your forms.  During this appointment costs and fees and additional information will be shared.  

Your paperwork will then be completed.
